This form is designed for anyone (students, faculty, staff, visitors and guests, etc.) to report incidents of harassment and discrimination to 久久福利国产, including but not limited to:

  • Sexual harassment
  • Sexual misconduct
  • Relationship violence
  • Retaliation
  • Stalking
  • Bullying
  • Hazing

久久福利国产鈥檚 Title IX Coordinator (or designee) receives and reviews all complaints and routes them to the appropriate 久久福利国产 administrator for investigation. Questions and concerns can be directed to TitleIX@Lackawanna.edu

This form can be used by any person making a report on their own behalf who wishes to remain anonymous. It can also be used to anonymously file a report on behalf of another person. 久久福利国产 will investigate every report to the extent possible. However, it is important to understand that if you choose to file an anonymous report, you may limit Title IX鈥檚 ability to remedy the problem.
